Within 30 days. After 30 days a 1.5% service charge will be charged to the account every month the balance is carried. We do accept VISA, MasterCard, Discover, and AMEX. Cash and personal checks are also accepted. There is a $20.00 fee for a returned check. There is a $50.00 charge for broken appointments and for appointments cancelled within a 24 hour notice.
For patients of record with no insurance, we require 50% down at the time services are rendered. Balances can be paid off in 3 equal payments over 3 months. A 1.5% service charge is applied to call accounts over 30 days until balance is paid in full. Patients with insurance should start paying their co-pay starting with the date of service in 3 equal payments over a 3-month period.
We file your insurance as a courtesy, however all charges are your responsibility the date the services are rendered. Many insurances have a limited time frame to send in claims so if your insurance has changed, please provide us the updated information as soon as possible, as insurances take a while to inform us that there has been a change in your coverage.
